Affidavit of the Master or Commanding Officer or First or Second Officer I II Officer of the Cincinnati from Genova do solemnly, sincerely, and truly swear that I have caused the surgeon of said vessel sailing therewith or the surgeon employed by the owners thereof to make a physical and oral examination of each and all of the aliens named in the forgoing Lists or Manifest Sheets 29 in number, and that from the report of said surgeon and from my own investigation, I believe that no one of the said aliens is an idiot, or imbecile, or a feeble minded person, or insane person, or a pauper, or is likely to become a public charge, or is afflicted with tuberculosis, or with a loathsome or dangerous contagious disease, or is a person who has been convicted of or who admits having committed a felony or other crime or misdemeanor involving moral turpitude or is a polygamist or one admitting belief in the practice of polygamy or an anarchist, or under promise or agreement express or implied to perform labor in the United States, or a prostitute, or a woman or girl coming to the united states for the purpose of prostitution, or for any other immoral purpose, and that also, according to the best of my knowledge and belief, the information in the said List or Manifests concerning each of said aliens named therein is correct and true in every respect.

Zeaulieu, Surgeon of the SS Cincinnati sailing therewith, to solemnly, sincerely and truly swear that I have had 12 years' experience as a Physician and Surgeon, and that I am entitled to practice as such by and under the authority of the Yenusen Geouvineuh and that I have made a personal examination of each of the aliens named hereto, and that the forgoing Lists or Manifest Sheets 2 in number, are, according to the best of my knowledge and belief, full, correct, and true in all particulars relative to the mental and physical condition of such aliens.
Sworn to before me this 17th day of April, 1913 at New York.

This Manifest indicates the ship left Genova on 2 April, Naples on 3 April, and Palermo on 4 April. Recorders used the word do, or ditto, from number 97 to 103, 109 to 111, 113 and 114 for able to read; and 97 to 114 for nationality, race and last address. Passengers 97 to 99, then 101 to 105, then 107 to 109, and 111 to 114 have a thick vertical line for repeated destinations. Transcriber replaced all dittos and lines with the information referred to. Passengers have a check to the right of the age column and by Final Destination columns. Passengers 89, 92, 93, 100, 106, and 110 have no check, as they are crossed out. This usually means, that passenger was not on this ship. Passengers 101 to 114, Fiore to De Luttio, all stamped NON IMM for Non Immigrant most lightly. No passengers above so stamped. This ISTG page is made up of several partially filled pages of the manuscript. Passengers 86 and 87 are on one page, 88 to 95 on another page, and 96 to 114 on another page. Passengers 96 to 114, a page number, 26, is stamped next to given name column, but 24 stamped at end of page. Numbers 16 and 16 T are written below recorders name, in last address and final destination columns. Unable to make out recorders name. The passenger listing above is only one page of each page of the manifest. Both pages maybe seen on the Ellis Island Web Site. The Italian language has no letter j, k, or w. Bain is first of two passengers on this page. 87 Carter, copy is very light, letters choppy. Underneath given name is word with a line drawn through it, looks like Lpsrry, Lfisrry or Ltiorry. Initial looks like E but maybe G. Age looks like 00 written very small, maybe 60 or 66, or ??. Occupation was left blank, Last Address, Port Colbo?na maybe Colbonia, Colborna or Colborva. Nearest Relative, looks like Mrs Wrift or Wright, first letter looks written over several times, and that address, looks like 555 Tme de Cedoflita or Cedefeita, nothing found like it, Oporto but not clear. Atlas shows a Port Colborne in Ontario Canada. Stamped at end of line is Non Imm, for non immigrant. This is the last of two passengers on this page, below names of page is stamped 8, and 2 a and 2 t written on right side. Signed below is the name V G Macintosh, Inspector. 88 to 95, above in left corner is written the number 9, next to right margin between lines of Name in Full and Family Name is number 33, Below that, between Given Name and Years columns is stamp of number 3, on bottom of page, below permanent address is written the number 5, and below it is IT or 1T, and then at bottom of page is stamped the number 9. 88 Zla?o?rolsky, Copy is very light, letters choppy, hard to read. Surname looks like Zlasofrolsky, maybe Zlasopolsky, Zlatopolsky or Zlatofrolsky as Nearest Relative looks. F?aak, looks like Fsaak but maybe Fsaale. Last Address ?ao Paulo, probably Sao Paulo, though looks like Tao. Nearest Relative address of father looks like 4 or R E 118 St East. Passengers is first of 8 of this page. 89 ?, ?, all columns lined out, blocked by a thick line, usually means person did not sail on this ship.
