Affidavit of the Master or Commanding Officer, or First or Second Officer

I, JOHN JOHNSTON, MASTER, of the ITALIA, from NAPLES, do solemnly, sincerely, and truly DECLARE that I have caused the surgeon of said vessel sailing therewith, or the surgeon employed by the owners thereof, to make a physical and oral examination of each and all of the aliens named in the foregoing Lists or Manifest Sheets, ___* in number, and that from the report of said surgeon and from my own investigation, I believe that no one of said aliens is an idiot, or imbecile, or a feeble-minded person, or insane person, or a pauper, or is likely to become a public charge, or is afflicted with tuberculosis or with a loathsome or dangerous contagious disease, or is a person who has been convicted of, or who admits having committed a felony or other crime or misdemeanor involving moral turpitude, or is a polygamist or one admitting belief in the practice of polygamy, or an anarchist, or under promise or agreement, express or implied, to perform labor in the United States, or a prostitute, or a woman or girl coming to the United States for the purpose of prostitution, or for any other immoral purpose, and that also, according to the best of my knowledge and belief, the information in said Lists or Manifests concerning each of said aliens named therein is correct and true in every respect. I, LUIGI MARINO, Surgeon of the ITALIA, sailing therewith, do solemnly, sincerely, and truly declare that I have had 14 years' experience as a Physician and Surgeon, and that I am entitled to practice as such by and under the authority of Naples University & Boston, Mass: USA, and that I have made a personal examination of each of the aliens named herein, and that the foregoing Lists or Manifest Sheets, ___* in number, are, according to the best of my knowledge and belief, full, correct, and true in all particulars, relative to the mental and physical condition of such aliens. *  The use of an * indicates an omission or error made by
   the original recorder. The notes below apply to the list
   as a whole; notes concerning individuals are included
   at the end of their respective paragraphs, above.

?  The use of a ? indicates a word or letters that could not
   be read, due to quality of original document. Notes 
   referring to these question marks are included at the end
   of the individual paragraphs, above.

*  Much of this transcript was typewritten, making the names
   very clear, but the information for columns 8 to 10 was
   handwritten. 

*  In several instances, names are given for only one family
   member, though that member may have been accompanied by
   others.  For example, Passenger 28 was accompanied by two
   unnamed sisters, for whom no further information is given.
   We only know that they were over the age of one year, and
   the rest of the data regarding disposition, discharge, and
   meals apparently refers to all the members of the group. 

*  In many cases, only a street address is given, with no 
   indication of what city or state it was in.  Usually, if
   it was necessary to continue the address on another line,
   the recorder would write it above the original line. However,
   in the case of Passengers 12 and 13, there seemed to be an 
   extra line, and I cannot tell to whom it belongs: Superior,
   Cleveland, O.  It is even more confusing because of the name 
   "Liberto" which was written in, unnumbered, above Passenger 13.
