Transcriber's Notes:
* The use of an * indicates an omission or error made by the original
  recorder.
* The use of a ? indicates a word or letters that could not be read, due to
  quality of original document.
* REPORT... bur? this looks like a very faint "t". The rest of the word
  is off the page. 5? this looks like the pharmacist's symbol for ounce. /??
  could be an open nine or the letter "g" while the second ? looks like a five
  or an "s".
* Mrs Thompson - In the Occupation column is a very faint word. The first
  ? suggests an "m" or "w". The second and third ?? are same height as
  m's and n's and u's and r's
* Ja? Dixon - could be a "p"
* J Byron? - the pen makes an upwards curve, I suspect this is an "s".
* J Brun?li? - the first ? is a blank space possibly from the copy
  machine. The second ? is the start of a letter, a short downstroke.
* In the Occupation column there is a marking which looks like a short
  slightly tilted "4". The first one is noted above the G in Gentleman.
  They are probably check marks which have been crossed.
* There were no deaths nor births.
