Transcriber's Notes:

* The use of an * indicates an omission or error made by the original recorder.
* The use of a ? indicates a word or letters that could not be read, due to quality of original document.
* The right side of this list was cut off during the original microfilming process. Thus not all information on that side was readable.
* The second line is overstamped "Seattle, Wash." Port Townsend, W???? is presumed to be Port Townsend, Washington.
* Names were written with first name or title first.
* In the column entitled "Age," a space is indicated for age in months. However, none were indicated. Therefore, this column was eliminated.
* In the column entitled "Last Place of Residence," all responses were "Hong Kong." Therefore, this column was eliminated.
* In the column entitled "Complexion," all responses were "Olive." Therefore, this column was eliminated.
* In the column entitled "Color of Eyes," all responses were "Dark." Therefore, this column was eliminated.
